I’m working on trying to get all of our items we collect within the “Vitals” area of our product with LOINC codes. This is the one that I’m having the hardest time finding something for.
From our client request for it: “<span style=“background-color: #ffffdd; font-family: Verdana, sans-serif; font-size: 12px;”>Oxygen Requirement - it is needed to complete our CMNs (Certificate of Medical Necessity) for home oxygen use. A physician’s certification of medical necessity for oxygen equipment must include the results of specific testing before coverage can be determined. Claims for oxygen must also be supported by medical documentation in the patient’s record. Separate documentation is used with electronic billing. This documentation may be in the form of a prescription written by the patient’s attending physician who has recently examined the patient (normally within a month of the start of therapy) and must specify: A diagnosis of the disease requiring home use of oxygen”</span>
The parts of this are:
<ul style=“margin-bottom: 1em; font-family: Verdana, sans-serif; font-size: 12px; background-color: #ffffdd;”>
The oxygen flow rate; and
An estimate of the frequency, duration of use (e.g., 2 liters per minute, 10 minutes per hour, 12 hours per day), and duration of need (e.g., 6 months or lifetime).
I am wondering if this might have been implemented incorrectly here (trying to do it all in one field), I am seeing these LOINCs for some parts which might make more sense to collect separately:
I’m not seeing as many good things for frequency or duration of use/need. I see a simplified one based on the answer list here: https://loinc.org/88658-0 , although answer lists are always “optional”, it seems like maybe that could be used as a good code for their second part?
Thank you for the question. I’ve used Oxygen Flow Rate 3151-8 in the past when the recording of supplemental oxygen flow is noted. It would be an additional piece of information to accompany some pulmonary tests/gases/O2Sat results, as it gives an impact on how the patient is able to make use of the supplemental O2. The order panel it’s contained in is https://loinc.org/55415-4/ .
This would be opposed to ventilator settings, where the dial in settings are recorded as well as the patient’s blood gases to indicate utilization of the ventilated oxygen. To see these, use system:ventilator in search.loinc.org
As I read your need, I wonder about https://loinc.org/88658-0/ Use of Supplemental Oxygen for clinician’s initial order. If you would like to share the current mappings for the certificate of necessity and let the LOINC team see where the deficiencies lay, you may be able to get more exact terms issued. email me at pdbanning@mmm.com for a sidebar conversation, if you’d like.
