I can help with some of your questions. Regenstrief Institute presently releases updates to LOINC in February and August each year. Each new version of LOINC contains both new and revised terms. The volume of each varies by release. Revised terms are generally to correct small errors or clarify modeling. It is never our intent to change the meaning of a concept once it is introduced. There are fields within the LOINC database which indicate changes. There are also separate release artifacts which track new and updates terms.
New and revised content is updated in our LOINC Terminology Service using HL7 FHIR on the same day the terminology is published. For instance, Verizon 2.82 was released on Feb. 24, 2026 and our FHIR API had the new information within a few hours of the download file being published.
