Flowsheets & CLIA Certification

We currently have several outside laboratory integrations into our EHR. We are struggling with the best way to display these outside lab results in a typical flowsheet without having to manually curate a list. We (EHR) have an automated process that runs to pull in the LOINC code mappings for all lab interface setups. However, when resulting, providers aren’t familiar with the LOINC code/names. They would prefer to see the Reported Codes. Customers are concerned that they could lose their COLA accreditation/CLIA certification if they were to create custom test panels with arbitrary codes. Is loss of COLA accreditation/CLIA certification a possibility from using arbitrary codes? Do you have any suggestions on how to display these lab results in flowsheets? Thank you!

The LOINC codes/names shouldn’t be used for provider facing displays. Typically the performing laboratory with determine their test order and result names to which the LOINC code is mapped (and used in messaging, backend databases, etc.) It is ok to use the LOINC name for these databases for reference, and quality checking.

Which codes are you referring as “reported codes?” The unique ID for each test in a performing laboratories messaging and compendium, which many refer to as “local codes” or something else?

A similar question arose with FHIR US Core laboratory data where the LOINC is used for the codeable concept. We asked CMS CLIA if the test order or result (test to be performed in CLIA language for the order or test performed for the result) could be represented by the LOINC code (or any other code or symbol alone). They indicated the test name needs to be included. US Core FHIR has been updated with test name as a “must support” item.

Before adopting LOINC, how did you display lab results in your flow sheets? Are you able to “receive” the performing lab’s test name and then route it to your flow sheet for your providers?
