F10 versus f449

Hi Everyone,

I am trying to map Ses i 1 (f449) IgE to LOINC but could not find any matches. LOINC 100350-8 includes f10 under Related Names but not f449. ThermoFisher’s website has their f449 mapped to LOINC 100350-8 and f10 to LOINC 6242-2.

Do I need to request a new LOINC for f449 or is it ok to map it to LOINC 100350-8?

Thank you!

Welcome!

Can you confirm whether your test result has quantitative result values or Seminquantitative values like RAST testing? Is the ImmunoCap IVD test used to produce your result values? You mentioned Thermo Fisher, so assume you may be referencing: f449 Ses i 1 | Thermo Fisher Scientific

It is correct the f010 is the whole sesame seed and includes multiple aspects of which Ses i is one of them. I’d map it to 100350-8. We see both in this reflex test from LabCorp: 607740: Allergen Profile, Sesame, IgE With Component Reflex | Labcorp

The LOINC team may need to add f449 as a synonym to 100350-8 to help folks (sometimes the related names list doesn’t include all the synonyms so I wouldn’t use that as an absolute decision maker.) Also given 100350-8 is a newer code, there is likely not enough data yet to include it in the top LOINC rankings. Before it was released, the best LOINC option would have been 6242-2, to which many mapped their Sesame IgE order or result. It’s good to hear folks are mapping to 100350-8, but if some haven’t yet updated their maps they still could be using 6242-2, and which would be on historic data before 100350-8 was released.

The LOINC code you want is indeed 100350-8. Ses i 1 is present as a related name but you are correct that the allergen code is missing. Note there is some confusion in terms of the allergy content in LOINC which I previously brought up. The term with ending in RAST class was deprecated and replaced with another term with a method of RAST and a scale of SemiQN. “RAST” is an obsolete term for a test for specific IgE performed by RIA. 102124-5 Sesamum indicum 1 IgE Ab [Presence] in Serum by Radioallergosorbent test (RAST) actually represents the RAST class and should not have a method. It should also not be an orderable as it is simply calculated as a semiquantitative unit fro 0-VI based on a range of values. It is also considered by Thermo Fisher to be superfluous in light of quantitative results.
