History and physical exam sections

I am looking at these in RELMA using the class hierarchies and I have several questions:

1. LP 7800-8 is History Catagories (sp?); was it intended that there be any subcategories or just the codes as a flat list?
2. A substantial number of the history codes appear to be observed findings vs reported e.g. skin appearance (39104-2 and 5) and all the wound description codes; I would have expected to find such codes under Physical Exam Sections and Measures
3. Similar to Q1, was Physical Exam Sections and Measures intended to have subcategories or just the codes as a flat list?

I will volunteer to do subcategories for H and P if anyone thinks that's appropriate.
